Riseboro Community Partnership and NYU Langone Brooklyn PPS have partnered to address the issue of chronic asthma in New York City.
From February 1st through April 30th RiseBoro Community Partnership will be conducting Asthma Home Assessments throughout all five boroughs. We are limiting the program to 1000 participants. Those that participate will receive air purifiers (while supply lasts) as well as information and referrals to help best manage their care.
Our goal is to meet with individuals who suffer from chronic Asthma and provide them a fresh eye of their home environment and find ways to reduce asthma triggers.
Effective management of environmental triggers in the home can reduce the number and severity of an individual’s asthma episodes. This reduction may result in fewer emergency room visits and hospitalization, increasing a person’s quality of life and reducing health plans’ cost of care.
To qualify you must have been diagnosed by a physician with asthma and currently receive Medicaid assistance.
